Information Governance

Information governance is the manner in which the Trust handles information, in particular personal and sensitive information relating to individuals i.e. staff and service users. It brings together a framework of standards, best practices, legislation and Codes of Practice to ensure that all information is:-

  • Held securely and confidentially;
  • Obtained fairly and efficiently;
  • Recorded accurately and reliably;
  • Used effectively and ethically;
  • Shared appropriately and lawfully.

 Our policies and procedures follow a legal framework which encompasses the following areas:-

  • General Data Protection Regulation;
  • Freedom of Information 2000;
  • Environmental Information Regulations 2004;
  • The NHS Code of Confidentiality;
  • Information Security; 
  • Records Management (including health records);
  • Access to Health Records Act 1990;
  • Data Quality; 
  • The Caldicott Reviews.

The framework ensures the integrity, the confidentiality and security of the information we process. It provides a clear and consistent structure of the range of rules that all NHS employees are required to adhere to. 

By adhering to this framework it helps the Trust to safeguard patient confidentiality and empower staff to perform in their role efficiently using appropriate key information governance principles. Furthermore, it helps us to:-

  • Provide excellent care and treatment to our service users;
  • Comply with the law;
  • Fulfil our IG Toolkit requirements; and
  • Provide improvements to our information governance agenda.

Requests for Information

Under the General Data Protection Regulation, all living individuals or ‘Data Subjects’ have a right to find out what information is held about them. Should you wish to submit a Subject Access Request to find out what information is held about you please refer to the Health Records section for further details. 

The Freedom of Information (FoI) Act 2000 requires the Trust to be accountable in publishing corporate records of our structures, policies, procedures and decision making processes which are of public interest. For further details about this please refer to the FOI section

Raising a Concern

Should you have a concern about any aspect of your care or treatment at the Trust or how your information has been handled then please raise an informal complaint with the Patient Advice and Liaison Service (PALS)

Information Commissioner

Should you remain dissatisfied with any information governance aspect at the Trust you can pursue your complaint with the Information Commissioner’s Office. The ICO is the UK’s independent body that regulates the use of personal data in accordance with the Data Protection Act 1998 and the Freedom of Information Act 2000.
