Children’s Community Nursing Team
About this service
Our aim is to prevent and shorten hospital admissions for all children aged 0-19 years of age who have a Gateshead GP by promoting Family Centred Care.
To provide Specialist Children’s Nursing Care to the children 7 days per week service
We see the children either at home, relative’s homes, nurseries, mainstream and special schools, respite facilities, and summer play schemes.
What you can expect from this service
When a referral is received, the team will carry out a triage telephone call with the child’s family and arrange the initial home visit. During the phone call, information about equipment required and any worries that family may have will be gathered. The team will ensure that they have the appropriate equipment and information required in order to carry out the initial visit.
If a child requires routine assessments they may need to attend clinic for these, but where this is difficult alternative arrangements can be made. Families and health professionals can contact the team for advice at any time.
Who is in the team?
The team comprises children’s nurses and children’s nurse assistants. Some qualified staff within the team are trained in Paediatric Clinical Assessment Skills and Independent and Supplementary Nurse Prescribing.

Children and young people registered with a Gateshead GP who require specialist nursing care
How to be seen by this service
Referrals are made to this service via a GP or other health professional.
This can be done initially via telephone 0191 2834660 and then followed up with an emailed referral to Gan-tr.gatesheadccnt@nhs.net
When is this service available?
Monday – Friday 8.00am – 8.00pm
Saturday, Sunday and Bank Holidays 8.00am – 5.00pm
