About the service
The discharge liaison team support complex discharges from hospital back to the community.
The team work closely with the hospital social work team in facilitating hospital discharges and offer advice and support for the wards around discharge.
The team also liaise with community support services and agencies and provide discharge workshops.
The team are based within a discharge hub that comprises of the discharge liaison team, the hospital social work team and administration support.
The discharge liaison team are based at the Tranwell unit at the QE Hospital, Gateshead.
