About the service

We provide post diagnostic support and treatment for patients aged 65 and over who have been diagnosed with dementia. We also deliver Cognitive Stimulation Therapy. Our aim is to assist patients and carers to enable them to live well with dementia.

What you can expect from the service

When we receive a referral a member of the clinical team makes telephone contact to discuss the referral, explain the pathway, arrange a convenient appointment and answer any queries. Patients are seen in the Memory Hub at Bensham Hospital or can be visited at home.

How you are referred to the service

A GP will refer to Gateshead older person’s community mental health services. The referral will then be triaged to a Consultant Psychiatrist for diagnosis. If a diagnosis of a dementia is given then the consultant, will refer to the Memory Hub for treatment and post diagnostic support.

Who is in the team?

The team is predominately nurse led with support from an Occupational Therapist and clinically overseen by a consultant psychiatrist. All of our nurses are registered mental health nurses.

Eligibility Criteria

To be eligible for this service, you will be aged 65 and over with a diagnosis of dementia. Referrals are only accepted by old age psychiatrists within the Trust.

When is this service available?

This service is available Monday – Friday, between 9am – 5pm at Bensham hospital.
