Hospice at Home

The hospice at home service is to provide additional support of the existing community nursing teams. This service operates on a needs-led basis to support people to be cared for and die at home if that is their wish.

About the service

The Gateshead Hospice at Home Service aims to support adults who have a life limiting illness and enable them to be cared for, and die, at home, if that is their choice, regardless of diagnosis. 

The service works on a needs-led basis to support individuals who, without intervention, would require an unnecessary admission to an acute hospital ward due to crisis. 

They facilitate hospital discharges for individuals who require immediate domiciliary care support. Health and social needs will be met by aligning the service with nursing, therapy services and named clinicians, and continue to support families into bereavement via signposting to external services. 

The Hospice at Home service is a dedicated team of Registered Nurses, Health Care Assistants (HCA) and Administration Support staff.

Where is the service based?

The team are based within Elmwood, Queen Elizabeth Hospital, operate 7 days a week and offer care 24/7.
